WV Performance Effectiveness Review Tool (WV PERT)

The West Virginia Bureau for Public Health / School-Based Health Continuous Quality Improvement Program and its process seeks to assure that quality medical, psychosocial and educational health services are delivered in accordance with current best practices. Under this program each SBHC performs an annual internal self-evaluation using the Program Effectiveness Review Tool (PERT), which includes an evaluation of the center’s compliance with the Principles, Standards and Guidelines for School-Based Health Centers in West Virginia.

To help you begin thinking about the chart audit component of the WV PERT, the WVSBHA Communication’s Team is posting the various indicators for you to download, review and decide which one you may choose for your self-evaluation.

• Each SBHC will pick one clinical quality indicator to focus on per year.
• Ten charts will be selected to audit. This year will be your baseline audit.
• The WV PERT is meant to be helpful and is in no way punitive.
• Technical assistance is available as needed.
